How do I enrol in Benefits?

A step-by-step guide to enrolling in benefits as an employee.

Welcome to Collage!

As a new hire to your company, Collage is designed to capture your personal information for both payroll and benefits enrolment.

Step 1: Complete your personal information

Step 2: Fill out Emergency Contact info

Step 3: Add your benefits enrollment info

Step 4: Add any dependents (i.e. spouse/children)

If you select YES to either spouse/children, a drop down will appear for you to enter their information. Please make sure that all details are entered correctly! This information will be sent to your company's insurance company to register you and your family!

When adding a spouse you will be asked to provide details for their benefits coverage. If they do not have coverage currently, select None.

When adding dependent children you will be asked to provide basic personal information:

Step 5: Select your coverage preferences

If you have added a spouse or dependent you will be able to elect for Single or Family coverage.

Step 6: Download and complete your Beneficiary Assignment Form. Once completed a paper copy of this form must be returned to your company's Plan Administrator.
