Your guide to using Collage to manage your benefits

Disclaimer: This only applies if your organization has Benefits Sync through Collage.

Use Collage to:

  • Update your personal information

  • View your benefits coverage and benefits booklet

  • Update dependent information

  • Add or remove a spouse or dependent child(ren) to your plan

  • Update dependent student status

  • Any other dependent information updates

Use your insurance provider to:

  • Submit your claims for reimbursement

  • Download your benefits card

  • View coverage balances

Changes you make in Collage will be updated with your insurance provider.

You can add dependents or update their information right from Collage.

  • Click Benefits on the left-hand side navigation

  • Select My Enrolment

  • Click Make Changes

  • Enter the required data and follow the prompts to update to your dependent information

  • Click Submit

You can add your spouse or update their information right from Collage.

  • Click Benefits on the left-hand side navigation

  • Select My Enrolment

  • Click Make Changes

  • Enter the required data and follow the prompts to enter/update your spousal information.

  • If you’re adding spousal coverage for the first time, you’ll be prompted to elect Family coverage now that you have a dependent on the plan

  • Click Submit

Use Collage to update the coverage type for Extended Health Care and Dental (i.e. change from Single to Family coverage).

  • Click Benefits on the left-hand side navigation

  • Select My Enrolment

  • Click Make Changes

  • Select Other from the drop-down list of reasons

  • Write in the reason for your change in coverage status (this helps us to ensure that your update is allowed for compliance reasons)

  • Follow the prompts to complete the update

  • Click Submit

Use Collage to update your over-age dependents coverage by adding their post-secondary education information.

  • Click Benefits on the left-hand side navigation

  • Select My Enrolment

  • Click Make Changes

  • Select Change in Student Status from the drop-down list

  • If applicable, review your spouse information. Then, click Yes to the question “Is this dependent a student between the ages of 21 and 25 who is attending an educational institution....”

  • Under the Coverage Details section, confirm your coverage election type

  • Once finished, click Submit

Where can I find the beneficiary assignment form?

  • Click Benefits on the left-hand side navigation

  • Select My Enrolment

  • Download and fill out the form

  • Once completed, give it to your employer’s plan administrator

When should I update my beneficiary assignment form?

This form should be updated for any events of your life such as:

  • Marriage

  • Change of spouse/ divorce

  • Birth of a child, change of dependents, adoption

  • The unfortunate death of your previous beneficiaries

Glossary

Beneficiary

A designated individual who may become entitled to a benefit under a plan in the event the plan member becomes deceased.

Dependent

A person who relies on another, especially a family member, for financial support. Common examples include spouses or partners, children, or even siblings.

Effective Date

The date in which the benefits plan becomes effective and employees can begin submitting Claims.

Eligibility Date

The date in which an employee becomes eligible to enrol in the benefits plan.

Probation period

The period of time between your date of eligibility and your effective date .
